A Home Care Package is a consumer directed package of services tailored to meet your specific care needs. Packages are offered by the government on the basis of availability from a centralised quota of packages.
There are four levels of packages available which are designed to suit your care needs now and into the future as your needs increase.
- Home Care Level 1 – to support people with basic care needs.
- Home Care Level 2 – to support people with low level care needs.
- Home Care Level 3 – to support people with intermediate care needs.
- Home Care Level 4 – to support people with high care needs.

- Initial ACAT assessment

Who is eligible?
Individuals can refer themselves to the ACAT for assessment or be referred by family, friends, general practitioners or allied health professionals.
Eligibility for a Home Care Package needs to be determined by an Aged Care Assessment Team (ACAT).

How does the program work?
It starts with an assessment by your local Aged Care Assessment Team (ACAT).
The assessment process consists of a series of questions and you will not be charged for visits by the ACAT team members.
This ACAT assessment process determines your eligibility and also determines the level of Home Care Package you qualify for.
You then go into a national queue to wait allocation.
Thereafter you will be advised you have been allocated a home care package.
The next step is to appoint a home care package provider to begin providing services.
The provider will develop a care and support plan in consultation with you and if appropriate, your family or carer.

What is the cost?
The Federal Government pays the bulk of the costs associated with a Home Care Package, however you are asked to make a contribution to the cost of the care dependant on your income. We can discuss this with you in more detail.
