Consumer Directed Care (CDC)

Consumer Directed Care is the new way of delivering government funded Home Care Packages. It allows you to have choice and control over the type of care you or your loved one receive, how it is delivered and who delivers it. Of course the level of control and choice can be varied.

Our aim at ESP Healthcare is to provide a high level of service right across our organisation. From our carers, our co ordination staff, case management, accounts team and management.

What are CDC Home Care Packages?

Home Care Packages provide coordinated support when assistance with day-to-day living is required.

A Home Care Package is a coordinated package of services tailored to meet your specific care needs. Packages are offered on the basis of availability and the ability of the program to meet your needs. The package is funded by the Australian Government and now you have more choices with the Consumer Directed Care initiative.

The objectives of the Home Care Packages Program are:

  • To assist people to remain living at home for as long as possible; and
  • To enable consumers to have choice and flexibility in the way that care and support is provided at home

There are four levels of packages available which are designed to suit your care needs now and into the future as your needs increase.

  • Home Care Level 1 – to support people with basic care needs.
  • Home Care Level 2 – to support people with low level care needs.
  • Home Care Level 3 – to support people with intermediate care needs.
  • Home Care Level 4 – to support people with high care needs.

If you have a CDC Home Care Package, ESP Healthcare can provide services direct to you. We have service agreements (or are preferred providers) with most major Home Care Package providers.

Who is eligible?

Eligibility for a Home Care Package needs to be determined by an Aged Care Assessment Team (ACAT). Individuals can refer themselves to the ACAT for assessment or be referred by family, friends, general practitioners or allied health professionals.

The assessment process consists of a series of questions and you will not be charged for visits by the ACAT team members.

How does the program work?

Once you have been assessed by the ACAT, our Home Care Packages Coordinator will visit you at home to discuss your needs. The care and support plan will be developed in consultation with you and if appropriate, your family or carer. What is the cost?

The Federal Government pays the bulk of the costs associated with a Home Care Package, however you are asked to make a contribution to the cost of the care, if you can afford to do so – dependant on your income.

Government Funded Home Care Packages

If you are considering applying for in home care assistance, ESP Healthcare can provide services direct to you as we have service agreements or are preferred providers with most major government Home Care Package providers.

We can assist you through this process of applying for a Home Care Package and put you in touch with the Aged Care Assessment Team (ACAT) to be assessed for eligibility.

We can then provide government funded services where you are eligible and private services where you are not.

  • ESP Healthcare is also proud to be an approved provider to the government disability “Community Support Program” (CSP).
  • We have also been independently verified for compliance to the NSW Disability Service Standards.

Policies and Procedures
All ESP Healthcare policies and procedures are consistent with the Home Care Standards and the NSW Disability Service Standards.


ESP Healthcare has been assessed and registered by JAS-ANZ and International Standards Certifications. We comply with the requirements of the International Standard 9001:2015 for the provision of home care assistance to clients living in the community through the delivery of personal care, respite care, shopping, transport, meal preparation, domestic assistance and case management.

Frequently Asked Questions

We try to match each client with the most appropriate care worker. We endeavour to ensure this care worker continues providing services to you, subject to your approval and his or her availability.

We are flexible as to the services you may require. Should you require changes to your rostered care services contact the ESP Healthcare office on (02) 9818 8004.

We provide a letter to you setting out our service agreement terms and engagement details following the initial consultation.

We understand your day-to-day circumstances may change at short notice. We try to give you as much service-flexibility as possible but if a service is cancelled after 5.00pm the day before the service or on the same day the service is due you will be charged a cancellation fee up to the first four hours of that shift.

We invoice for our services weekly and our payment terms are 14 days. The invoice details the services provided each day.

Invoices are emailed or posted. Payment can be made via EFT or cheque.

Our fees for carer services will depend upon the care services that are needed. Our fee schedule can be made available on request. We can help you decide what level of service will best suit the person in care.

Our main phone number – (02) 9818 8004 – is answered by ESP Healthcare staff 24 hours a day, 7 days a week.

Our offices are staffed from 7.00am to 6.00pm Monday to Friday. Out of these hours, calls are diverted to our on-call personnel.

All ESP Healthcare staff are fully covered under the following insurance contracts:

  • $20,000,000 under Public and Products Liability Insurance
  • $20,000,000 in aggregate and $10,000,000 in any one claim under Civil Liability and Professional Indemnity Insurance
  • Employees are fully covered with Workers Compensation Insurance under the Workers Compensation Act
All staff employed by ESP Healthcare have had Australian Federal Police checks and where working with clients under 18, a Working with Children check is carried out.
